Documents required to receive Christmas Assistance:

:: ​State ID or valid driver's license for all adults in household

:: Identification for each child in the household:

:: Birth certificate, school record, DWSS card

:: Social Security card for each member of the household or DWSS Card listing everyone in the household

:: Proof of Residency: mail that has your name and address, utility bill (water, phone, gas or power) or rent receipt

:: Must provide income information for the entire household
