Documents Needed for Services

On your initial visit, please bring:

Picture I.D. for all Adults

Current Utility Bill (Water, Gas or Power ) in your name

-OR-

Current lease with all family Members on it and a current rent receipt

Birth Certificate or custodial papers for each child

Social Security cards for all family members

Proof of all household income

If you do not have a Social Security Card or a Tax ID number, you must have a valid Nevada State ID or Driver's license along with all the other required documents in order to receive assistance.

If you live with someone, you must have two pieces of mail sent to you at that address, postmarked within two weeks and our residence verification form with a copy of their current utility bill along with all the other required documents.



Return Visits:

You will receive a code number on your initial visit.  To qualify for subsequent services, you will need to provide your code number, ID and proof of current residence.
